Use the Sample Transfer Tracking Form to enter information about sample transfer from the processing laboratory storage location at IU Simon Cancer Center to the research laboratory storage locations and Purdue University and IUSCC. Samples have been collected from consented patients in the Colorectal Cancer Pilot Study and processed at IUSCC. This form tracks information associated with the transfer of samples from IUSCC to Bindley Biosciences Center for temporary storage,. The form is restricted to authorized members of the CCE Sample Acquisition Group.

This form tracks information associated with the storage of samples at Purdue University before their transfer to the laboratory. The form is restricted to authorized members of the CCE Sample Acquisition Group.

Use the Sample Hand-Off Tracking Form to enter information about sample distribution to the target research laboratory. Samples have been collected from consented patients in the Colorectal Cancer OMIC Pilot Study, processed at IUSCC, transferred to Bindley Biosciences Center for temporary storage, and handed off to the OMIC laboratory for analysis. This form tracks information associated with the hand-off of the stored sample to the laboratory. The form is restricted to authorized members of the CCE Sample Acquisition Group.
