Blood Sample Acquisition and Tracking
Sample tracking information is collected for consented patients enrolled in the Colorectal Cancer Pilot Study funded by the Department of Defense. Samples are collected at IU Hospital, IU Cancer Center, Wishard Hospital and IU Health Arnett Hospital. All samples are processed at IU Cancer Center. Sample aliquots are distributed to "omic" laboratories at IU Medical School, IU Bloomington, and Purdue University. IUCC Sample Receipt

Use the IUCC Sample Receipt Tracking Form to track the receipt of samples from IUHA to IUCC .

Sample Transfer
|
Use the Sample Transfer Tracking Form to track sample transfer
from IUCC to Distribution Centers, including Bindley, IU Medical School, IU Bloomington and U of Washington.

Sample Storage
|
Use the Sample Storage Tracking Form for blood samples stored at Bindley Biosciences Center . Bindley Biosciences Center tracks sample storage for samples (1) transferred from IUCC and stored prior to hand-off to the target research laboratories at Purdue University, and (2) returned from the Omic Labs after use, for permanent storage.

Sample Hand-Off

Use the Sample Hand-Off Tracking Form to enter information about sample distribution from Purdue Bindley Bioscience Center to the target research laboratories at Purdue University.

Labs Sample Receipt
|
Use the Sample Receipt Form to enter information about samples received at IU Medical Center (for the Xu Laboratory), IU Bloomington (for the Klaunig Laboratory) and U of Washington (for the Raftery Laboratory).

Labs Sample Return
|
Use the Sample Return Tracking Form to select blood samples that will be returned to Purdue Bindley Biosciences Center from the Purdue Labs. Bindley Biosciences Center tracks aliquots returned from the research laboratories at Purdue University, and these will later be stored by Bindley using the Sample Storage Form.
