Patient Data Collection and Blood Sample Acquisition PDF Print E-mail
Clinical and sample tracking information is collected for consented patients enrolled in the Colorectal Cancer Pilot Study funded by the Department of Defense. Clinical data is collected from questionnaires, patient records, pathology reports, and physician notes. Samples are collected at IU Hospital, IU Cancer Center, Wishard Hospital and IU Health Arnett Hospital. All samples are processed at IU Cancer Center. Sample aliquots are distributed to "omic" laboratories at IU Medical School, IU Bloomington, and Purdue University. Patient Diagnosis
Use the Patient Diagnosis Form to enter diagnosis data and information about previous tumor staging, surgery and treatment.
Patient Treatment
Use the Patient Treatment Form to enter information about patient therapies and patient treatment status.

Patient Lifestyle
Use the Patient Lifestyle Form to enter demographic, health, diet, tobacco/alcohol use, employment, and environmental information.
  • Demographic Data Viewer for viewing and searching patient demographic data.
  • Smoking and Alcohol Data Viewer for viewing and searching patient smoking and alcohol-related information.
  • Lifestyle Questionnaire for patient demographics, tobacco/alcohol use, health history, employment history, environment factors and other data that will be used in analyzing and modeling patient biological data.
Patient Behavior Lifestyle
Use the Patient Behavior Lifestyle Form to enter physical activity, stress, social support, organizational and quality of life information.
Sample Transfer
Use the Sample Transfer Tracking Form to enter information about sample transfer from the processing laboratory storage location at IU Cancer Center to the research laboratory storage locations.
Sample Storage
Use the Sample Storage Tracking Form to enter information about sample storage at Purdue Bindley Biosciences Center prior to hand-off to the target research laboratories at Purdue University.
Sample Hand-Off
Use the Sample Hand-Off Tracking Form to enter information about sample distribution from Purdue Bindley Bioscience Center to the target research laboratories at Purdue University.
Sample Receipt
Use the Sample Receipt Form to enter information about samples received at IU Medical Center (for the Xu Laboratory) and IU Bloomington (for the Klaunig Laboratory).
